Castro Valley Companion Animal Hospital offers patient forms online so you can complete them in the convenience of your own home or office. Don’t forget to bring them with you to your appointment!
Surgical Information Packet – Required
These forms are only necessary when your pet will be undergoing a surgical or sedated procedure. The first form gives us your consent to give your pet everything he/she needs for safe treatment. Going under anesthesia or sedation can be dangerous, so the second form serves as a precaution. If anything were to happen during your pet’s procedure, would you want a doctor to perform CPR? You can complete these at home and save yourself time when you drop your pet off with us in the morning.
Drop Off Consent Form
If you have scheduled a drop-off appointment with us and need to be in and out as fast as possible, feel free to complete this form at home. You can bring it with you when you drop your fur baby off with us. The information on this form lets us know the history and current state of your pet’s health. What questions, concerns, goals, regarding wellness can we help you with? Make sure to provide us with as much information as possible. This form acts as your replacement when the doctor is asking all the necessary questions they need to know to make your pet feel better. Of course, if we ever need any additional information, we will always give you a call.
Acutherapy Forms
When you schedule your consultation appointment with Dr. Gandolfi for Acutherapy, you will be required to fill out all 3 of these forms. The answers to these questions give Dr. G the necessary tools to help identify how he should treat your pet.
